The HealthConnection Clinic in North Vancouver, a collaboration between the North Shore Division and Vancouver Coastal Health, was highlighted by the College of Family Physicians of Canada (CFPC) in the second issue of their Innovation in Primary Care series. Published in March 2018, Innovation in Primary Care: Caring for Unattached and Marginalized Patients outlines the success of the clinic, a team-based facility combining the expertise of GPs, nurse practitioners, registered nurses, and social workers. The HealthConnection Clinic opened in 2013 and each year provides care to over 900 vulnerable patients, many of whom face the challenges of mental illness, substance use, or homelessness.
This is the second division-related feature in the CFPC’s innovation report. The first issue – Innovation in Primary Care: Effective Primary/Secondary Care Interface – was released in January 2017 and included an overview of the Fraser Northwest Division’s role in developing Pathways.
