The Comox Valley Primary Care Network includes Family Physicians in Patient Medical Homes (PMHs) along with nurse practitioners, nurses, pharmacists, mental health counsellors, social workers, physiotherapists, Indigenous wellness advocates, dieticians, and others working to provide team-based care across Family Physician clinics in the Comox Valley and surrounding area.
