What is the Long-term Care Initiative (LTCI)?

The Comox Valley Long-Term Care Initiative (LTCI) works to improve the care of all residents in LTC homes, and to improve the experience of care providers. It works to address the challenges in LTC and supports implementing best practices for medical care by collaborating with Family Physicians (FPs), Nurse Practitioners (NPs), care home teams, and health authority leadership to develop system-level changes to the culture & structure of LTC.  

In July 2024, LTCI funding from the Family FPSC  was re-distributed to introduce On-Call Availability Payments (OCAP) to compensate eligible FPs for after hours on-call shifts. The Division coordinates an after-hours call group integrating FPs and NPs into one shared coverage approach to ensure 24/7 availability for LTC homes when urgent issues arise.

The LTCI also conducts quality improvement work and offers relevant educational opportunities. 

LTCI Working Group 

The Comox Valley LTCI is supported by a working group with members from the Comox Valley Division of Family Practice, leadership and staff from all care homes, and Island Health.

How to get involved in LTC

  1. Review the LTCI Letter of Agreement   
  2. Reach out with questions to LTCI@comoxvalleydivision.ca 
  3. When ready, sign the LTCI Letter of Agreement and submit to LTCI@comoxvalleydivision.ca 

Long-term Care and the Family Practice Services Committee

As led by the FPSC, LTCI is designed to support FP’s and NP’s to develop local, scalable, and sustainable solutions that target meeting the following five best practice expectations and three system-level outcomes: 

Best Practice Expectations 

  • 24/7 availability and on-site attendance, when required 
  • Reduced cost per patient as a result of a higher quality of care 
  • Improved patient & provider experiences 
  • Reduced unnecessary or inappropriate hospital transfers 

System-Level Outcomes 

  • Attendance at case conferences 
  • Completed documentation 
  • Meaningful medication reviews 
  • Proactive visits to residents
