Our Projects
KB Division helps practitioners meet patient & practice needs, leads change in an evolving primary care system, & works with partners to shape a better future for health care in Kootenay Boundary, & BC. Projects are aimed within our four strategic pillars: Clinic Supports, Team Based Care, Clinic Operations, & Engaged Community. We also host & administrate regional Shared Care projects. To provide a suite of supports to clinics, including digital health, financial optimization and back-end HR, and to share information between PSP, KB Division, and PCN leadership staff members interacting directly with Primary Care clinics to ensure optimized supports, streamlined communications for our clinicians, while avoiding unnecessary duplication of work. |

To support clinics to optimize use of EMRs and videoconferencing, ensure access to specialist information and support clinics in communicating with their patients via the provision of a suite of digital health supports. |

Emergency Preparedness |
To enable a faster, more thorough clinic emergency response for continuity of patient care/clinic operations through: i) support to family practice and specialist clinics to prepare emergency preparedness plans, and ii) the development of micro-crisis and incident command response plans that will enable the KB Division to respond to and support clinic emergencies. |

MOA Supports |
To improve Medical Office Assistant (MOA) work satisfaction by building capacity through clinic efficiencies and by offering wellness activities. An indirect goal is to see strong retention of KB MOAs achieved in part through project activities. |

Selkirk Rural Pre-Medicine Bursary |
To support aspiring rural medical professionals within the Selkirk College Rural Pre-Med program via a scholarship bursary and funding to KB Rural Residency Program for student and faculty supports. |

Measurement System for Physician Quality Improvement (QI) Pilot |
To explore, in partnership with the provincial Measurement System for Physician QI program, how the use of primary care data sets from private clinics and health authorities can be utilized to improve primary care, and to document and understand the governance and administrative processes required to enable the secure extraction, sharing, and linking of those data sets. |

PACC works with partners including the BC Patient Voices Network, KB Division, Interior Health, the Collaborative Services Committee, and more. Participation is inclusive and open to all KB residents |

Chronic Pain |
To strengthen the regional network of care for people living with chronic pain, incorporating a tiered system of care through: i) supporting the re-start up of the Kootenay Boundary Regional Hospital Pain Medicine Clinic, ii) developing and/or promoting resources, tools and information for both chronic pain care providers and patients. |

Gender Affirming Care |
To develop strategies to empower local primary care and specialist providers to coordinate gender affirming care for patients with diverse gender identities, and to access resources available to support them in this. To work with Trans Care BC to promote educational opportunities and awareness of provincial resources available for health professionals and patients. |

To develop in-practice Adverse Childhood Experiences (ACEs) history taking and poverty intervention tools, build provider skills and confidence, and support the sharing of SDH information between Family Practitioners and Specialists. |

To improve the cancer patient care experience through: i) improved communication processes, ii) improved coordination between care providers, iii) increased understanding of cancer care pathways, and iv) increased networking opportunities for care providers. | |
Telepsychiatry |
To initiate early mental health assessment and intervention for patients presenting with psychiatric symptoms at the emergency department (ER), while admitted to medical floor at Kootenay Lake (KLH) and Boundary District (BDH) Hospitals, or in long term care through a pilot project providing urgent virtual consults with psychiatrists. |

Complex Medical Frail |
To support development of team-based care for older adults with complex medical conditions and/or frailty and develop prototypes to assist IH with Specialized Care Services Programs (SCSP) implementation, with a particular focus on improved communication processes and coordination between family physicians and specialists, and between physicians, home health nurses and inter-professional teams. |

To expand the use of in-clinic and facility-based telehealth systems available region-wide for use in shared consultations across all members of the patient’s care team that is easy to use, cost effective and secure. Emergency Medicine Network- Extension Funding |
To support the KB Emergency Medicine Network in continuing to develop and enhance relationships across all emergency departments, deliver quality improvement projects that are identified by the Network, and provide support for the second Emergency Medicine Network Retreat. |
