The Live Well campaign, along with the tools and resources on this page, was developed by the Kootenay Boundary Shared Care Palliative Care project. This initiative brought together local primary care and specialist physicians, allied health providers, and patient and health authority partners to guide the process. It was supported by the Shared Care Committee, a partnership between Doctors of BC and the Government of British Columbia.
