Attachment Hub Coordinator
Up to $25 an hour - Full-time

We are seeking a full-time Attachment Hub Coordinator as a part of our team to help attach patients in our region who are in need of a primary care provider. This position will also support some event coordination. The Attachment Hub Coordinator is Monday to Friday from 9 a.m. to 5 p.m. in the Division office in New Westminster for a minimum of 3 days per week. This position reports directly to the Primary Care Network Director.

Patient Medical Home (PMH) Coordinator
$65,000 a year - Full-time

The Patient Medical Home (PMH) Coordinator’s role is to assist family practices in practice efficiency optimization. This is a full-time position with a flexible schedule that involves working in the Division office, throughout the FNW region, and remotely. This role reports directly to the Program Director.

Program Director
$95,000–$105,000 a year - Full-time, Fixed term contract

The Director is one of 4 leadership positions within the Division and is responsible for overseeing and managing a specific set of programs. The primary role is to ensure the successful planning, implementation, and evaluation of programs to achieve the Division and the funders goals and objectives. This role provides oversight on all Division Shared Care projects, Patient Medical Home supports and services and the organization's evaluation and reporting of all programs and projects. This position is full-time with a flexible schedule that involves working in the Division office, throughout the FNW region, and remotely.

Healthcare Project Coordinator
Up to $65,000 a year - Full-time

The project/program coordinator supports the program and project teams with administrative and organizational expertise required for programs. They help to make sure everything that is required and needed for programs is addressed. Coordinators are familiar with all aspects of the FNW programs they support, including short and long-term goals.
