Shared Care Project: Heart Failure

The Heart Failure project aimed to increase collaboration and communication between SND Family Physicians, NPs, Cardiologists, and Outpatient Heart Function Clinic (HFC) leadership, ultimately improving heart failure patients’ health outcomes.
The primary focus was to integrate SND Family Physicians into the HFC care pathway, thus improving the transition of care between HFC specialists and FPs. To this end, the working group held a series of CME sessions to support FPs in better understanding the patient journey at the HFC, along with an integrated care pathway pilot.
Project Activities Included:
- A pilot of an integrated care pathway at the Heart Function Clinic that introduced two FP follow-up appointments during the patient journey at HFC
- Two in-person CME events and a virtual lunchtime learning session supporting connection and understanding between community FPs and HFC staff and processes
