Primary Care Network (PCN)

To support a clinical network of primary care providers, focused on team-based care with co-located GPs, NPs, Allied Health and other clinical staff working together to increase patient attachment and improve patient care, access, and cultural safety in Kootenay Boundary.

Future of Clinics: Community Health Clinics (CHC)

To explore options around supporting the evolution of smaller private practice clinics to larger self-managing clinics offering team-based care, that are more actively connected to their local communities and networked via a supportive hub that provides economies of scale, equity and quality improvement.

CHC Webinar Series >

PRIMA Health
To deliver PRIMA Health, an independent, Division governed stepping stone to a regional community health centre network. Prima Health has already begun to provide management & administrative services to one KB clinic, will “soft-launch” the KB Health Online regional virtual clinic later this year, and is exploring a demonstration clinic to act as an incubator for transferable, member-driven health care innovation in 2022.

COVID Response
To implement a range of responses relating to the COVID-19 pandemic including: emergency preparedness in clinics, provider COVID meetings, vaccine rollout, an MOA locum project, evaluation of a permanent locum position, and participation in ISC virtual care and COVID working groups. Chronic Pain

Gender Affirming Care

To better understand the needs of gender diverse people in our region including access to health care issues, and to develop strategies to empower local primary care and specialist providers to better coordinate gender affirming care in Kootenay Boundary.

Social Determinants of Health 
To embed SDH and ACES in-practice screening in several test clinics and systematize the integration of SDH data into EMRs. Screening data will also be shared with SPs to improve  poverty and trauma informed care practices.

Regional Oncology
To support the development of a regional team-based care Oncology approach, with a particular focus on improved communication processes and coordination between cancer care providers across Kootenay Boundary.

Telepsychiatry
To enable virtual psychiatric consults for patients who present with psychiatric symptoms at emergency departments or during their admission to medical floors at hospitals

Telemedicine

Complex Medical Frail

 

To support development of team-based care for older adults with complex medical conditions and/or frailty, and chronic health conditions and develop prototypes to assist IH with Specialized Community Services Program implementation.
