The Surrey-North Delta Primary Care Network Social Work Team addresses social determinants of health for people with a variety of challenges.
The program exists to support Family Physicians and Nurse Practitioners in Surrey-North Delta to meet their patients' broader health needs. The vision of the PCN Social Work Program is to be a family practice extender, with PCN social workers operating alongside FPs and NPs as part of the patient medical home team to provide wrap-around primary care for patients.
Referral Information
PCN Social Workers offer brief, focused interventions for mild to moderate challenges aimed at preventing situations from becoming more severe.
If you are unsure whether a patient's situation warrants a PCN Social Work Program referral, we recommend making the referral - a PCN social worker will be able to determine whether or not that patient is a good fit for the program. Unsure? Refer!
- Acceptance Criteria

- 19 years or older
- not active with another community social worker or case manager
- referred by and attached to a Family Physician or Nurse Practitioner practicing in Surrey-North Delta
