What Are We Asking For and Why?

As part of our PCN commitment, we are required to report our community’s attachment numbers to the Ministry of Health. This information will also allow us to more accurately assess our collective capacity to attach patients so that we can make informed decisions about how best to support attachment efforts going forward.

Demonstrating SND’s commitment to patient attachment will ensure that we continue to receive the funding we need to build our PCN services and supports.  

We encourage all our FP and NP members to share these numbers – this small step will make a big difference for our community and everyone who participates will have the chance to win a prize!

Currently, we are collecting data for the dates from April 1, 2025 to March 31, 2026. 

 

Your Privacy is Paramount

The attachment numbers we gather will be anonymized and aggregated to reflect only the total number of new attachments in Surrey-North Delta. No patient names or information will be collected. The information will be used solely for the purposes of these PCN reports; we will not be sharing individual data, nor will we be evaluating any individual Family Physician or Nurse Practitioner's attachment numbers.

We recognize that attachment numbers will vary significantly between individuals for a variety of reasons.

Didn't I already do this?

Yes. This is our fourth Attachment Roundup. In order to keep our attachment data up to date and ensure its reliability, we need to gather these numbers annually. This happens in April of each year, to collect annual numbers. (April is the beginning of our fiscal year.)

Do I need to do this if I'm billing the attachment code (98990)?

Yes. Every FP should be billing code 98990 for every new attachment. This is to ensure the patient is reflected on your panel vs. showing up as “pending” on multiple FP/NP pending lists. This will support provincial efforts to ensure every patient is connected to an FP or NP. In SND, we are taking the additional step of this Attachment Roundup to gather accurate community-level data so we are asking everyone to participate.

I'm on an LFP or NTP contract and I already use an attachment code. Do I still need to do this?

We have been made aware of several situations where there was a discrepancy between an NTP physician's attachment numbers and the data that the ministry has collected. We ask that you share your attachment numbers with us even if you are using the correct attachment codes - this will help us to accurately assess our community's progress and needs in addition to supporting our FPs and NPs in the case of a discrepancy.

Can my MOA do this for me?

Yes. You can share the appropriate cheat sheet with your MOA and request that they complete the query and submit the number on your behalf. We recommend that you understand what your attachment number is for your own information. 

I am part of a group practice - can we submit our numbers together?

Yes. However, please ensure that every member of the practice has given permission to share their attachment number prior to submission. Please also indicate which Family Physicians and/or Nurse Practitioners are included in the submission.

How can I maximize my chances of winning?

Multiple draws will be held throughout the month of April and everyone who doesn't win will be entered into subsequent draws. That means, the sooner you submit your attachment numbers, the more chances you have to win!
