Dizziness or Vertigo Survey
We want to better understand your lived experiences with dizziness or vertigo related symptoms and conditions, and how you access and navigate the health care system in the Fraser Northwest (this includes New Westminster & Tri-Cities). Your answers will help to inform the work with our community of patients and health care providers to determine the challenges and to identify ways in which we can improve the healthcare accessibility for patients with dizziness or vertigo related symptoms and conditions.
Dermatology Survey
This survey targets patients who have dermatology healthcare needs in New Westminster or the Tri-Cities. It seeks to improve the continuity of care in dermatology services.
Please take a few minutes to complete this survey. This survey should take 5-8 minutes to complete. Your answers will help to inform the work with our community of patients and healthcare providers to determine the challenges and to identify ways in which we can improve the continuity of care provided as it relates to dermatology services.

Blood Clot in Veins Survey
We want to better understand your healthcare experiences and challenges accessing care with Deep Vein Thrombosis at the Emergency department. Deep Vein Thrombosis (DVT) happens when a blood clot forms inside a deep vein in your body, usually in your legs or pelvis. Veins are like tubes that carry blood back to your heart. Sometimes, for different reasons, a clot can form and block the normal flow of blood in these veins. This can cause pain, swelling and warmth in the affected area.

ER Follow-Up Survey
We want to better understand the primary healthcare experiences of patients who visit the emergency department in New Westminster or Tri-Cities.
We are surveying patients who have lived experience in order to help us to develop actions locally to improve transitions in care between the hospital and where you get the majority of your primary care in the community (Family Doctor's office, Walk-in Clinic, etc..)

Maternity Postpartum Survey
We are currently conducting a survey of the accessibility of care for postpartum people and their babies living in the New Westminister and Tri-Cities area. We want to ask you about your experience accessing services and supports after labour and delivery. This survey should take 5-8 minutes to complete.

Older Adult Survey
Older adults with complex health issues require significant levels of services which often include their family physician, specialist physicians, family caregivers, community health services, and acute care. Coordinating this care can be a significant challenge for patients, families, and providers. Issues often arise relating to communication, roles and responsibilities, access to services, care plans, information sharing and more. These challenges present several opportunities to improve how we provide more effective patient and family-centred care.
We are currently conducting a survey about the care that older adults receive in New Westminster and/or the Tri-Cities. From this information, we will work with our community of patients and healthcare providers to determine ways in which we can enhance the care of older adult patients with complex and multiple health issues.

Alcohol Use Survey
We recognize that there may be a great deal of barriers and geographic location is one of them, so this survey is applicable to anyone living in BC who feels that they want to share their feedback. Our local grassroots efforts will impact the Primary Care Providers and Programs serving New Westminster & Tri-Cities with the hope to share what we learn with our colleagues around the province.
In this survey, we are looking to learn more about the experiences patients/public may have in their healthcare interactions that are either impacted by or related to alcohol drinking. Our local health care providers in the New Westminster and Tri-Cities communities want to reduce the fear of judgement and stigma and improve access for people living in the community who are seeking help for alcohol use. We need feedback from people with lived experience to help identify the barriers and hear ideas for improvement. This is your chance to have a say in your local healthcare.
