Patient Surveys for the Fraser Northwest & New Westminster Region
The Fraser Northwest Division of Family Practice (FNWD) is working with community physicians, specialists, health care providers, and stakeholders to better understand health services and care provided in New Westminster and the Tri-Cities.
The purpose of these surveys is to collect and analyze descriptive data which represents the perceptions and experiences of the people accessing services in New Westminster and Tri-Cities. Responses to the surveys will enable continued and ongoing program design and implementation in ways that reflect the views and concerns of the people they intend to benefit.
We’d like to hear from you about your experiences of receiving care for various health-related concerns such as:
- Chronic Pain
- Maternity Care
- Mental Health
- Palliative Care
- Older Adult
- Geriatric Psychiatry
- Women's Health
- Women’s Health
- Cancer Care
- And a general overview of accessing healthcare and potential barriers
The surveys will be open until November 30th and each survey you complete counts as one entry into a draw to win a $50 gift card.
Fraser Northwest Division of Family Practice is one of 35 divisions in British Columbia that make up the Divisions of Family Practice, an initiative of the Family Practice Services Committee, a partnership between the Province of B.C. and Doctors of B.C. Fraser Northwest Division of Family Practice currently has over 400 members that provide primary care services in Anmore, Belcarra, Coquitlam, New Westminster, Port Coquitlam and Port Moody.
