Chilliwack Division of Family Practice

Primary Care Clinical Pharmacists

Primary Care Clinical Pharmacists (PCCPs) provide comprehensive medication management services focused on the on-going care of adult patients with complex conditions to prevent and resolve medication-related problems. PCCPs also provide education to patients about their medications and address barriers to adherence.

PCCPs are a part of the PCN Chronic Conditions team that will work together to support the individual's overall health.

Examples of areas that a PCCP can support include:

  • Identifying and resolving medication issues
  • Multiple drug therapies or health conditions
  • Optimize drug therapy for chronic diseases
  • Polypharmacy concerns
  • Medication allergies or adverse reactions
  • A complete picture of how an individual's medications are working, including prescription, non-prescription, supplements and natural health products
