Occupational Therapy
A PCN Occupational Therapist (OT) considers how psychological states, cognitive capacity, motor skills, environment, communication abilities, habits and motivations, and readiness for change intersect with a person’s health challenges. By using a variety of interventions, OT’s aim to improve or maintain a client’s participation and engagement in activities of everyday life.
The PCN Occupational Therapist is a part of the PCN Chronic Conditions team that work together to support the individual's overall health.
Examples of areas that the PCN Occupational Therapist can support include:
- Helping clients understand chronic conditions in the context of their environment and life roles
- Self-management strategies for functional management of health conditions
- Re-engagement in daily routines and habits that are affected by health and well-being
- A wide variety of psychosocial, physical, emotional and cognitive assessments
Eligibility
- Individuals must be referred by their family physician or nurse practitioner
- 18 years of age or older
