Chronic Disease Nurse

PCN Chronic Disease Nurses offer short-term self-management support for mild to moderate chronic conditions. Treatment is based on patient needs and may include one-on-one support, group education and connections to local community resources. Services can be provided in-person or virtually depending on individual needs.

PCN Chronic Disease Nurses are a part of the PCN Chronic Disease team that work together to support the individual's overall health.

Examples of areas that a PCN Chronic Disease Nurse can support include:

  • Support and promotion of healthy lifestyle behaviours
  • Chronic conditions (e.g., diabetes, pain, COPD, heart disease, etc.)
  • Medication management
  • Goal setting
  • Interventional management
  • Follow-up assessments 
  • Pre-surgical optimization
  • Advanced care planning
  • Managing blood pressure

Eligibility

  • Individuals must be referred by their family physician or nurse practitioner
  • 18 years of age or older
