SHARED CARE PROJECT COORDINATOR

The Burnaby Division of Family Practice works with Shared Care (a Joint Collaborative Committee of Doctors of BC and the BC government) to support the collaboration between Family and Specialist Physicians and partners to improve the coordination of health care for patient populations.

The Position:
Reporting to the Shared Care Project Manager, the Shared Care Project Coordinator is responsible for leading the design, implementation, evaluation, quality improvement, risk management and progress reporting to effectively achieve the goals of Shared Care assigned projects. This position will work with the Burnaby Division of Family Practice, Fraser Health Authority staff, primary care providers (including fee-for-service physicians), specialist FPs and community agencies/organizations.
 
Key responsibilities:
  • Effectively engages and builds strong relationships with internal and external stakeholders.
  • Facilitate and coordinate meetings with working groups or task groups, including developing agendas, taking minutes, noting actions and conduct follow-ups to fulfill deliverables
  • Respond to requests for information from doctors, health authority and community in an efficient and courteous manner, providing explanations, direction, and instructions.
  • Leads development of project and grant proposals including research and budget components
  • Develops project plans and planning tools including workplans, Gantt charts, written reports and slide decks for different audiences
  • Leads implementation of projects through engagement with internal and external stakeholders including division members, staff, contractors, participating physicians, community organizations and health authority partners
  • Monitor project progress; recommend modifications to project plans as needed and lead any refinement processes.
  • Ensures delivery of the project is consistent with agreements, requirements and goals, and in compliance with policy and processes.
  • Coordinates the duties of the program(s) team(s) and working group(s) to fulfill the project deliverables and meet regularly to support, monitor progress and participate in project activities.
  • Ensures that the security and confidentiality of all data is maintained consistent with provincial regulations and policy.
  • Utilizes project data to prepare quarterly progress reports for submission and inform continuous quality improvement and refinement of each project. Document quality improvement processes and changes.
  • Prepares monthly and quarterly progress reports on all project activities
  • Meets regularly with the Director and/or Manager to confirm direction, to provide project updates, and to escalate issues or concerns that arise from the delivery of the project that may be of interest or importance to project teams and/or the Board of Directors
  • Researches and analyzes a wide range of qualitative and quantitative information from different sources to prepare or support the preparation of documents as needed.
  • Leads components of evaluation activities including development of indicators, development of data collection tools, data collection, data analysis, report writing, and dissemination of findings through data summaries, reports, slide decks and oral presentations.
  • Performs other duties/tasks as required or requested.
 
Qualifications:
  • A level of education, training, and experience equivalent to the completion of a bachelor’s degree plus a minimum of 3 years recent related experience with proven success in project coordination (preferably in health care). Master’s degree preferred.
  • Understanding of the healthcare system, primary care and residential care context.
  • Proven ability to manage and deliver projects.
  • Demonstrated skill and experience in community development and engagement, with experience in multi-stakeholder processes and evaluation.
  • Excellent interpersonal, communication and organizational skills.
  • Flexibility and the temperament and skills to manage change in a rapidly evolving community health initiative with a significantly compressed timeline.
  • Demonstrated ability to be innovative, creative, and solution seeking.
  • Experience in managing projects focused on the primary care sector and working with family physicians is an asset.
  • Demonstrated ability to coordinate, plan, implement, organize, and problem solve.
  • Excellent time management and organizational skills.
  • Must be able to work well under pressure at times to handle a wide variety of activities and confidential matters with discretion and professionalism.
  • Computer literacy with word-processing, spreadsheet, presentation and project management applications.
  • Ability to work independently, as well as work closely in a supportive capacity with Manager and or Director.
 
Salary Range: $67,000-72,000
