Burnaby Division of Family Practice

The Burnaby Division of Family Practice Board is currently comprised of five family doctors and two community representatives.

 

Dr. Leo Dai, Board Chair

Dr. Leo Dai, the current Chair of the Burnaby Division and a Board member for the past four years, has led key initiatives that have strengthened the organization and improved healthcare delivery in the community. His work has encompassed governance improvements, the development of Primary Care Networks (PCNs), Patient Medical Home (PMH) initiatives, interdivisional collaborations, and restructuring the organization to better serve its members. Throughout this journey, Dr. Dai has collaborated closely with partners such as the Ministry of Health (MoH), Doctors of BC (DoBC), Fraser Health Authority (FHA), and numerous community agency partners to ensure that initiatives align with the needs of the community and healthcare providers.
In addition to his role on the Board, Dr. Dai continues to practice as a longitudinal family physician, delivering patient care while managing a busy group practice. As a clinic owner, he understands the challenges and responsibilities of running a medical practice. He is deeply committed to integrating technology into healthcare while recognizing the importance of preserving the human element in medicine. Additionally, Dr. Dai is dedicated to making Burnaby an attractive destination for both locums and permanent physicians, recognizing this as vital to the sustainability of local practices.
Dr. Leo Dai is committed to ensuring that the Burnaby Division continues to meet the day-to-day needs of its members, providing the reliable support and resources that have always been a hallmark of the organization. Dr. Forson Chan, Vice Chair

Dr. Forson Chan (MD, CCFP) is the current Vice-Chair of the Burnaby Division and Board Member. He has been practicing in Burnaby since 2020 and founded Oakmount Medical in 2023. He is also a past District Delegate for Burnaby with BC Family Doctors, a locum at the Metrotown Urgent and Primary Care Centre, and clinical instructor as part of the UBC Faculty of Medicine. Dr Chan also provides consults for patients with substance use disorders and ADHD. 
Themes of Dr Chan's work over the past 3 years include: opposing the corporatization of medicine; tearing down technical and administrative barriers that get in the way of good medical care; and improving how doctors and patients can navigate our complex medical systems. Recent involvements have included: leading a grassroots bid for PHSA's Episodic Virtual Care contacts, Burnaby's shared-care project on substance use, projects on Artificial Intelligence and AI scribes with Doctors of BC, and the centralized referral initiative from PHSA. 
Dr. Forson Chan's career reflects a commitment to patient care, finding solutions to problems in the current medical system, and addressing community health challenges. He believes in empowering family physicians to do what they do best: providing excellent patient care. 

 

Dr. Jennifer Moore, Director

Dr. Jennifer Moore, a dedicated and experienced practitioner, has been a stalwart figure in Burnaby's healthcare landscape since 1994. Specializing in longitudinal family medicine, with a particular focus on obstetrics, Dr. Moore has witnessed the evolution of healthcare in Burnaby firsthand. A pivotal member of the Board of the Burnaby Division from its early stages, Dr. Moore played a key role in shaping the organization and has continued to contribute actively even after completing their term in 2017.
Dr. Moore is currently involved in various committees, including the Hastings Brentwood LLT, the PMH, and the PCN steering committees. This commitment reflects her ongoing dedication to the healthcare initiatives in Burnaby. Furthermore, she has been a driving force behind a Shared Care project on mental health, demonstrating a profound commitment to healthcare solutions.
Having attended the Leadership and Management Development Program through the FPSC during her previous tenure on the Board, she brings a wealth of knowledge and a strategic mindset to her role. This unique combination of formal training and practical experience positions her as an ideal candidate for continued service.
Beyond the professional realm, she is deeply passionate about her vocation and the Burnaby community. Her interactions with community players and health authority figures have enriched her understanding and strengthened her vision for the well-being of Burnaby residents.

 

Mr. Justin Boehringer, Director

Justin Boehringer joined the Burnaby Division of Family Practice Board in September 2025. He is a proud member of the Skeetchestn Band of the Secwépemc Nation through his father’s side, with white settler ancestry from his mother’s side. 
As Associate Director of Indigenous Perspectives at the new Simon Fraser University School of Medicine, Justin’s work focuses on advancing Indigenous rights, self-determination, and systemic change in medical education. He is committed to embedding Indigenous ways of knowing across curriculum, admissions, and governance, and to ensuring education serves historically marginalized and under-served communities. 
With the SFU School of Medicine focused on training future physicians in community-based primary care, Justin sees opportunities to strengthen connections between the school and local health systems. On the BDFP Board, he brings a collaborative spirit and a commitment to creating culturally safe, equity-driven, community-led health care solutions rooted in respect, reciprocity, and justice. 

 

Mr. Michel Pouliet, Director

Michel is a professional and skilled Executive Director with a wide array of skills and experiences in leading and managing organizations. He worked as the executive director of Burnaby Family Life for 7 years and that is where he connected with the work of the Division of Family Practice as he represented community agencies at the Primary Care Network Steering Committee.   
Michel is a passionate advocate for the needs of the most vulnerable members in our community with a strong track record of developing community partnerships. He is the Executive Director of Canadian Parents for French and focused on supporting French immersion and core French programs across the province and the Yukon. 

 

Ms. Simone Brandl, Director

Simone Brandl joined the Burnaby Division of Family Practice Board in September 2025. With over 20 years of experience working in community, Simone brings a deep understanding of the diverse needs of Burnaby’s population and a strong commitment to advancing health and wellness. 
As Executive Director of The Long Table, Simone has focused on increasing food security and reducing social isolation. During the 2020 pandemic, she developed a food systems program that led to the creation of nine community food hubs, which continue to support more than 12,000 people each week. This initiative helped inspire the City of Burnaby’s Food System Strategy, approved in 2024. For her leadership in this work, Simone was recognized as Burnaby’s Citizen of the Year. 
Simone has also served on numerous community associations and committees in roles such as Secretary, Treasurer, and Board member. With a systems-oriented approach and values-driven leadership, she is dedicated to ensuring that initiatives address both immediate needs and long-term health outcomes for the community. 

 

Dr. Tracy Monk, Director

Tracy Monk brings a wealth of experience and passion to the Burnaby Division of Family Practice, where she eagerly anticipates the opportunity to serve her colleagues and community. Having observed the innovative and forward-thinking initiatives of the Burnaby Division over the years, she is excited to contribute to this impactful work. 
Tracy's extensive leadership experience includes: 
  • Served six (6) years on as a representative on the Family Practice Services Committee (FPSC) 
  • Former President of the Society of General Practitioners (SGP, now known as BCFD) and many years of service on the SGP board 
  • Physician Lead of Pathways BC, a collaborative non-profit project initiated by Divisions & FPSC 
  • Inaugural Chair of the Fraser Northwest Division 
  • Former Head of the RCH Department of Family Practice 
  • Previous board member and current clinical data stewardship committee member of the Health Data Coalition 
Tracy's leadership roles have provided her with valuable insights and skills that she is excited to bring to the Burnaby board. Her office is located in the new Lougheed PCN, initially part of the Fraser Northwest Division. With significant changes planned for the area, she sees an exciting opportunity to shape the future of medical care in Burnaby and to align efforts with the broader Burnaby PCNs. Tracy has a deep appreciation for the Burnaby Division’s engagement with community partners. 
Tracy Monk is grateful for the opportunity to have served on the Board over the past year and remains eager to contribute to the continued success of the Burnaby Division of Family Practice. 

 

 

Dr. Vivian Liu, Director

Dr. Vivian Liu is honoured to be a Director and Treasurer on the Burnaby Division Board of Directors.  Having experienced the support of the Burnaby Division while transitioning into practice after completing her Family Medicine Residency, she felt welcomed and connected to her colleagues, patients, and services in her hometown.  Dr. Liu is now eager to give back and support new-to-practice Family Physicians and other medical professionals. 
As a Clinical Instructor at the UBC Department of Family Practice, the upcoming SFU School of Medicine, and a member of the Burnaby Division CME Committee, Dr. Liu is passionate about continuing medical education and fostering strong relationships with local specialist colleagues.  Through her work at the Edmonds UPCC and as a locum throughout Burnaby, she has gained an in-depth understanding of the unique needs of each Burnaby neighbourhood and recognizes the barriers patients, especially unattached ones, face when accessing care.  These insights have helped her highlight these needs as the Burnaby District Delegate on the BC Family Doctors Board of Directors. 
Dr. Liu has served on the Primary Medical Home for Priority Populations Task Group, where she contributed to designing workflows for the new clinic at the Edmonds Centre for Healthy Communities.  This clinic serves as the patient medical home for newcomers/refugees, expectant/new parents and their babies, people with complex mental health needs, and homeless/underhoused residents.  In addition, she has collaborated on Shared Care Projects and is currently the Family Physician Lead on the Shared Care Steering Committee for the Burnaby Screening for Unattached Patients Project, which aims to improve access to screening for metabolic conditions in these patients.
