Shared Care Portfolio Manager

The Burnaby Division of Family Practice is a non-profit Society governed by a Board of Directors comprised of family physicians. Our mission is to engage, support and mobilize family physicians on co-creating a network that will support the well-being of all members of the Burnaby community. The Division members are family physicians practicing in Burnaby, BC. We strive to be future-focused, with a commitment to innovative strategies and services that support our members and our community.
 
Position Background:
The Burnaby Division of Family Practice works with Shared Care (a Joint Collaborative Committee of Doctors of BC and the BC government) to support the collaboration between Family and Specialist Physicians and partners to improve the coordination of health care for patient populations. 
 
The Position:
Reporting to the Senior Director, Physician Engagement, Care Networks & Innovation, the Shared Care Portfolio Manager will: 
  • lead the design, implementation, evaluation, quality improvement, risk management and progress reporting to effectively achieve the goals of Shared Care projects. 
  • establish and maintain processes and frameworks that support the successful delivery of projects within, and beyond the Shared Care portfolio.
  • lead and mentor the Shared Care PMO team, promoting a culture of high performance, continuous learning, and a collaborative work environment. 
  • develop and maintain a framework for tracking project progress and outcomes, reporting regularly to the Senior Director, Physician Engagement, Care Networks & Innovation, and relevant stakeholders.
  • foster strong relationships with all relevant stakeholders, including Fraser Health Authority staff, primary care providers, specialist FPs, and community agencies/organizations.
 
Key Responsibilities:
  • Complete project plans and lead the implementation through engagement with division members, staff, contractors, participating physicians, community, and health authority stakeholders.
  • Monitor project progress and finances as required; recommend modifications to project plans and/or budgets as needed and lead any refinement processes.
  • Ensure delivery of the project is consistent with agreements, requirements and goals, and in compliance with policy and processes.
  • Coordinate the duties and responsibilities of the program(s) team(s) and working group(s) to fulfill the project deliverables, and meet regularly to provide support, monitor progress and participate in project activities.
  • Ensure comprehensive project evaluation by working closely with the project evaluator to ensure thorough and timely collection and analysis of data, and preparation of interim and final evaluation reports.
  • Ensure that the security and confidentiality of all data is maintained consistent with provincial regulations and policy.
  • Utilize project data to prepare quarterly progress reports for submission and inform continuous quality improvement and refinement of each project. Document quality improvement processes and changes.
  • Prepare a monthly progress report to the Senior Director on all project activities and assist to develop internal reporting forms and processes.
  • Meet regularly with the Senior Director to confirm direction, to provide project updates, and to escalate issues or concerns that arise from the delivery of the project that may be of interest or importance to project teams and/or the Board of Directors.
  • Evaluate project outcomes and processes to identify improvement opportunities. Implement changes that enhance efficiency, effectiveness, and stakeholder satisfaction across projects.
  • Develop the foundational structure and processes for a Project Management Office (PMO), ensuring it supports project delivery, aligns with the Divisions’ needs, and facilitates effective project management.
  • Provide guidance, mentorship, and identify professional development opportunities for project coordinators and other direct reports. 

Qualifications:
  • A level of education, training, and experience equivalent to the completion of a bachelor’s degree plus a minimum of 8 years recent related experience with proven success in project management (preferably in health care). Project Management Professional certification required.
  • Must have experience leading cross-functional teams and/or in establishing a Project Management Office (PMO).
  • Understanding of the healthcare system, primary care, and residential care context.
  • Demonstrated ability to successfully manage and deliver large scale projects.
  • Demonstrated skill and experience in community development and engagement, with experience in multi-stakeholder processes and evaluation.
  • Excellent interpersonal, communication and organizational skills.
  • Flexibility and the temperament and skills to manage change in a rapidly evolving community health initiative with a significantly compressed timeline.
  • Demonstrated ability to be innovative, creative, and solution seeking.
  • Experience in managing projects focused on the primary care sector and working with family physicians is an asset.
  • Demonstrated proficiency in coordinating, planning, executing, and problem-solving, with exceptional time management and organizational abilities.
  • Must be able to work well under pressure and to handle a wide variety of responsibilities with discretion and professionalism.
  • Ability to work independently, as well as work closely in a supportive capacity with the Senior Director.
 
Salary: $97,000 to $133,000 CAD
