Burnaby has activated three Family Practitioner (FP) networks.  FPs from the neighbourhoods of Edmonds, Metrotown and Brentwood/Hastings regularly meet to work together to identify and address patient needs at their neighbourhood level.  Burnaby’s PCN design makes provision for further integration of FP practices together to enable shared care across the practices, FP-to-FP support, and seamless exchange of patient information between practices.
