With improved collaboration between the Primary Care Network group members and the Fraser Health Authority Community Health Nurses, physicians feel they are partners in care planning, thus resulting in improved patient and provider satisfaction.

The community health nurse:

• Acts as the primary responsible nurse for a designated panel of patients
• Works in partnership with assigned GPs in providing seamless care and support to patients
• Establishes collaborative relationships with patients and families/caregivers and support them in managing their own care
• Collaborates with patients families/caregivers, primary care providers, inter-professional team members, acute care partners to coordinate patient care and develop a comprehensive shared care plan for all
