In anticipation of challenging times ahead due to COVID-19, it is important that we have conversations with our patients in advance of medical emergencies. Research tells us that patients and families want health care providers to initiate these conversations with them and that these discussions lessen anxiety and decrease unwanted and clinically non-beneficial interventions.
Advance Care Planning (ACP) and Serious Illness Conversations, which clarify goals of care and decision-making in terms of COVID-19, are especially important for high-risk patients. Please join us in an interactive conversation where we discuss the most recent ACP tools and assist family practitioners in initiating conversations around MOST documentation, SDM (substitute decision makers), improved communication between patients and their families, as well as an open Q+A period.
