Introduction & Accountability
The Surrey North Delta Division of Family Practice referred to in this Privacy Notice as “Division”, ("we," "us," or "our") is committed to protecting personal information in accordance with British Columbia’s Personal Information Protection Act (PIPA), and other applicable privacy requirements.
This privacy notice describes how the Surrey North Delta Division collects, uses and discloses personal information collected through the website and other electronic means (e.g. surveys conducted by the Division), as well as the rights website visitors and survey respondents have with respect to their privacy.
By visiting this website or by completing an online survey conducted by our Division, you are accepting the practices described in this notice.
Depending on the circumstances, the Division may collect, use, and disclose personal information for its own authorized purposes as an organization under PIPA. In other circumstances, the Division may collect, use, or disclose personal information on behalf of physicians, medical practices, Primary Care Networks, or partner organizations as a service provider, in accordance with applicable agreements, instructions, and legal requirements.
Our Privacy Officer(s) oversee our privacy management program, including policies, training, safeguards, incident response, and requests for access or correction.
1) Scope
This Privacy Notice applies to personal information collected through our website, interactions with our programs, events, services, and communications (such as email signups, event registration, surveys, and payment or reimbursement processes), and through in-person or other related communications.
This Notice does not apply to third-party websites or platforms that we do not own or control.
2) What Is Personal Information?
Personal information means information about an identifiable individual, such as name, contact details, payment information, or participation details. It does not include anonymized or aggregated information that cannot reasonably be used to identify an individual, or business contact information.
3) What Personal Information We Collect
The personal information we collect depends on how an individual interacts with us.
Information you provide directly may include contact information, registration and participation details, communications you send to us, and payment or reimbursement information where applicable.
Limited website and technical information may include IP address, browser type, device type, pages visited, and date and time of access, including through cookies or similar technologies.
We do not intentionally collect sensitive personal information through our website unless it is clearly explained and consent has been obtained. Where we process personal information on behalf of a physician, medical practice, Primary Care Network, or partner organization, the type of information processed will depend on the applicable program, service, initiative, agreement, or authorized purpose.
4) Purposes for Collection and Use
We collect and use personal information for purposes that a reasonable person would consider appropriate, in the circumstances and as permitted or required by the law, including communicating with individuals, administering and supporting programs and events, processing payments or reimbursements where applicable, maintaining appropriate records, and complying with legal, regulatory, accounting, and audit requirements.
5) Consent and Withdrawal
We collect, use, and disclose personal information with consent, except where otherwise permitted or required by law. Consent may be withdrawn, subject to legal or contractual restrictions and reasonable notice. If consent is withdrawn, we will explain the likely consequences where appropriate. To withdraw consent, please contact our Privacy Officer(s).
6) Use of Personal Information
We use personal information only for identified purposes or as otherwise permitted by law. Typical uses include administering programs, communicating requested updates, supporting physician engagement, conducting authorized analysis and evaluation, and complying with legal obligations.
7) Disclosure of Personal Information
Disclosures are limited to what is reasonably necessary, and we maintain reasonable safeguards.
8) Processing on Behalf of Physicians and Partners
In some circumstances, the Division processes personal information on behalf of physicians, medical practices, or partner organizations. In these cases, the Division acts as a service provider and handles personal information only for authorized purposes, subject to contractual, policy, and legal requirements.
9) Use of Technology and Cross-Border Processing
We use reputable technology and cloud service providers. Some personal information may be stored or processed outside British Columbia or Canada, including in the United States. Where this occurs, the information may be subject to the laws of that jurisdiction. We use contractual and other reasonable safeguards to protect personal information in accordance with PIPA.
10) Safeguards and Security
We use reasonable administrative, technical, and physical safeguards to protect personal information against unauthorized access, use, disclosure, or disposal. While no system is completely secure, we take reasonable steps to reduce risk.
11) Retention and Secure Destruction
Personal information is retained only as long as it is reasonably necessary to fulfill identified purposes and legal requirements. Information used to make decisions that directly affect an individual is retained for at least one year. When no longer required, information is securely destroyed or de-identified.
12) Access and Correction
Individuals may request access to or correction of their personal information, subject to limited exceptions under PIPA. Requests are handled by one of our Privacy Officer/s within the legislated timelines.
13) Children’s Privacy
Our website is not intended for children under 16, and we do not knowingly collect personal information from children without the appropriate consent.
14) Links to Third Party Websites
Our website may contain links to third party websites, platforms, or services that are not operated or controlled by the Division. We are not responsible for the content, privacy practices, or security practices of those third-party websites, platforms, or services. The inclusion of any link does not mean that the SND Division endorses the third-party website or service.

15) Changes to This Policy
This Privacy Policy may be updated from time to time to reflect changes in practices, technology, or legal requirements, or operational needs.
The current version will be posted with a revised date.
