This two-year initiative aims to increase the efficiency of matching prenatal patients with appropriate care providers by 50%. It addresses current challenges—including delayed access to care, duplicated services, and administrative burden—by supporting the implementation of a centralized intake system.
Patient stories illustrating systemic failures are paired with evidence from studies linking inadequate prenatal care to poor maternal and neonatal outcomes. The strategy aligns with a PCN-sponsored initiative to implement both an availability transparency tool and a central intake referral orchestration tool.
This system will also promote equity by including prompts for physicians to connect patients—particularly Indigenous patients—with culturally appropriate supports and services.
Ultimately the project will enhance provider experience, promote health equity, and improve patient access to timely care, while ensuring long-term sustainability. It will actively engage diverse partners, including Indigenous communities, to co-design a culturally safe and efficient referral process.
