Chronic Obstructive Pulmonary Disease (COPD) is a leading cause of emergency visits and hospitalizations, yet many flare-ups can be prevented through early recognition and timely action. 

This project addresses a key gap in how COPD care is managed across primary and acute settings: the inconsistent use and follow-up of written Flare-Up Action Plans. 

Building on strong evidence that these plans reduce the severity of exacerbations and prevent hospitalizations, our strategy focuses on increasing awareness and implementation among physicians and patients. 

By streamlining the creation and communication of Action Plans, we aim to improve early intervention, reduce avoidable ER visits, and empower patients and providers with the tools, knowledge, and coordination needed for better COPD self-management and continuity of care.
