As a Project Manager with Shared Care, Prabhjit leads the scoping, initiation, implementation, evaluation, and continuous improvement of Shared Care and Primary Care Network (PCN) projects at the Surrey-North Delta Division, ensuring each initiative achieves its goals effectively. She works closely with healthcare professionals and external partners to foster collaboration and engagement, while managing key project activities such as event design and delivery, stakeholder coordination, and project proposal development.
Prabhjit holds a Master of Health Sciences from Western University and a Bachelor of Science from Simon Fraser University. She is dedicated to advancing health equity by improving patient access to care and addressing the social determinants of health. Collaborative and strategic in her approach, Prabhjit thrives in dynamic environments and has partnered with health authorities and non-profit organizations to design and implement initiatives that strengthen service delivery and enhance health outcomes.
Outside of work, Prabhjit enjoys spending time with family and friends, exploring new restaurants, and traveling. She also loves staying active, whether at the gym or out on a hike.
