PCN Attachment Coordinator
As the Division's PCN Attachment Coordinator Monica's primary work is to facilitate patient attachment to a primary care provider -- either a Family Physician or a Nurse Practitioner -- and to support integration between primary care providers and community health resources.
Monica works with the PCN team, patient medical homes (primary care clinics), community health centers, Indigenous wellness centers, urgent and primary care centers, nurse practitioner clinics, and a broad range of community health care resources, organizations, and providers. As part of the PCN team, Monica works in partnership with Family Physicians, Nurse Practitioners and various PCN partners to design and implement an integrated community-wide attachment program.
Monica finds it very gratifying working in the healthcare environment and has worked in healthcare for over 20 years. She is passionate about helping patients and making a difference in their lives.
Outside of work, Monica is a proud mom of two teenage boys, whom she absolutely loves spending time with. She enjoys living an active healthy lifestyle, and spending early mornings at the gym – lifting weights! She also enjoys spending time with family and friends, travelling and baking.
