BC Centre on Substance Use (BCCSU)

 

The BC Centre on Substance Use is a provincially networked organization with a mandate to develop, help implement, and evaluate evidence-based approaches to substance use and addiction. The BCCSU’s mission is to provide provincial leadership in substance use and addiction research, education and clinical care guidance and to seamlessly integrate these pillars to help shape a comprehensive, connected system of treatment and care that reaches all British Columbians.

Bioped

 

BioPed Specializes in assessing and providing non-surgical treatment for conditions related to the functions of the feet and lower limbs, we work to alleviate discomfort and help people move through life with greater stability and peace of mind.

All BioPed Centres have on-site labs where we manufacture a full range of orthoses (orthotics) and provide clients with the convenience of adjustments and specialized fitting services. We also carry knee and lower limb braces – both custom and non-custom, compression stockings in various sizes, colours and compression grades as well as an outstanding collection of fashionable, orthotic-friendly footwear which can be customized if necessary for almost any foot irregularity.

CMHA – Bounce Back

Bounce Back is a free program of the Canadian Mental Health Association, offering evidence based tools and strategies to patients aged 15+ experiencing mild to moderate depression and/or anxiety. Participants work through workbooks and video clips to gain new skills and tools for better managing their mood, and have access to free phone coaching to support them as they work with these materials. We can supply Physicians and clinics with video link cards, DVD’s, and brochures to get their patients started, along with referral forms and information on how to refer their patients.

PCRS - Foundry

Pacific Community Resources Society (PCRS) has a 36-year history of providing a breadth of quality, accredited social services across British Columbia, including: education, employment, housing, substance use, mental health, and youth and family support services.  

PCRS is the host agency for Foundry Surrey is bringing together partner organizations that will address the health and wellness needs of young people aged 12 to 24 and their families. Through a partnership with individuals, businesses, other agencies, and governments, PCRS strives to achieve our mission of inspiring healthy and inclusive communities through leadership and collaboration. Foundry Surrey will offer young people 12-24 access to mental health and substance use support, primary care, peer support, and social services.

Surrey Overdose Response Community Action Team (CAT)

The purpose of the Surrey CAT is to act as a platform for collaboration, discussion and decision-making related to the opioid overdose response within the municipal boundaries of Surrey. The Surrey CAT members communicate, coordinate and deliver local projects in order to provide a robust strategic response to the needs of those most at risk of overdose. Indicators of success for the Surrey CAT will be a reduction in opioid overdose incidents and deaths.

The South Asian Shapedown Program

A holistic family centered healthy lifestyle program.  The program provides a comprehensive clinical assessment by a multi-disciplinary team consisting of a pediatrician, social worker and registered dietitian with  follow up healthy lifestyle  coaching and recommendations.  It is a 10-week group program of educational sessions covering topics related to healthy living that are child/teen or parent focused covering nutrition, mental health/wellness, family health and a physical activity component.  There is a no diet approach to the program.  The South Asian program provides a culturally responsive program that has been adapted and is delivered in Punjabi/English for South Asian families.

Speech and Hearing BC

A not-for-profit association with more than 1200 member speech-language pathologists and audiologists in British Columbia. We believe in the importance of providing treatment to individuals of all ages who may experience problems with their speech, language, hearing, voice, swallowing, fluency, and social communication.

Changing lives by improving communication. Our members provide assessment and treatment to individuals of all ages who may experience problems with their speech, language, hearing, voice, swallowing, fluency, or social communication.

Connecting those in need with those who can help. We connect people who have communication disorders with highly trained, certified professionals across BC who can help. Our website provides a listing of speech-language pathologists and audiologists who are certified members of our national affiliate, Speech-Language and Audiology Canada (SAC). Educating about speech, language, swallowing and hearing disorders. We provide educational resources for those wishing to find out more about a wide range of speech, language, and hearing disorders. Helpful links to online resources are listed in many sections of our website.

Advocating for the importance of public access to treatment services. We support advocacy efforts for continued access to speech, language, and hearing services.

The Surrey North-Delta Specialized Community Services Program (previously Home Health)

Offers community based care for seniors and adults with complex medical needs and/or frailty. We provide access for this population to various resources including: direct nursing care; allied health (OT, PT, Dietician, Clinical Pharmacist, SW); system navigation; access to long term care services (home supports, respite, MoH funded facility placement); and palliative care. Our objective is to provide a unified group of services working together so patients and caregivers can readily access what they need, when they need it through a team based care approach informed by client’s goals.

BC Family Doctors (formerly Society of General Practitioners)

BC Family Doctors’ purpose is to build an environment where family doctors thrive.

As the political and economic voice of family doctors in B.C., we are fierce and passionate advocates working to ensure family physicians are seen, heard and valued as the cornerstone of our health care system.

At BC Family Doctors, we represent you at tables where the voices of family doctors need to be heard. We update the family medicine fee schedule. Most importantly, we work to improve the system of care for patients and physicians.

Moving Forward Family Services

Moving Forward Family Services (MFFS) is a mentorship agency – providing counselling and social work practice opportunities to interns and new graduates beginning private practice. We receive funding for clinical supervision (supervisors who support interns and new graduates) and these supervisors in turn oversees an agency that has over 125 part-time counsellors, social workers and interns on post-secondary practicums, providing support to some of our most vulnerable communities. Community support also comes by way of donated space.

This unique model allows the agency to offer timely, affordable (including free for those with no income) counselling and support without being bound by restrictions based on gender, age, geography or presenting issues. It also allows MFFS to complement existing services (as opposed to competing with them for the same pools of traditional funding) thus reducing pressures on these services.  MFFS is committed to building and contributing to healthy, healing communities.

Health Data Coalition

The Health Data Coalition (HDC) is a physician-led data sharing initiative that encourages reflection and collaboration with colleagues and facilitates quality improvement in patient care though its application HDC Discover. It’s funded by GPSC, and powered by the enthusiasm and dedication of a growing network of family physicians from across the province. HDC’s primary focus is to empower physicians and clinics to understand and use their data for practice improvement. HDC Discover also provides a means to share data, if you choose, that is relevant for Primary Care Networks (PCNs) and other initiatives.  There is no cost to physicians to register in HDC Discover.

Family Caregivers of BC

The Family Caregivers of BC's Caregiver RX project can help health care professionals support family caregivers through education, outreach resources, and partnership.  The Caregiver RX project helps healthcare providers identify, engage, and refer family caregivers to support services in B.C with ease.  The Caregiver RX project can help health care providers with strategies to build partnerships with family caregivers to help expedite evaluation and prevent errors in care. 

Centre for Child Development

Since 1953, The Centre for Child Development of the Lower Mainland has worked to overcome societal barriers for those with disabilities and empower vulnerable children and families throughout the South Fraser Region.

We started as a grass-roots group of parents cooperatively organizing transportation to therapy sessions in Vancouver.

Throughout the decades, The Centre has expanded to providing comprehensive medical rehabilitation programs and multi-disciplinary therapeutic services to children with a wide range of abilities and conditions.  We now serve over 3,000 children each year through more than 30,000 sessions at over 286 locations in Surrey, White Rock, Delta, and Langley.

Many of the programs we offer provide unique services focused on inclusion, accessibility, and enhancing outcomes for children with a wide range of abilities.

Our Programs:

The Centre is proud to be the largest program in Western Canada to have consistently received 3-year accreditations from CARF since 2004 for our pediatric outpatient Medical Rehabilitation Services and Child & Youth Services.

Practice Support Program (PSP)

PSP’s full service coaching and mentoring approach helps physicians and teams build capacity in their practices. Our tailored practice supports enable physicians to better understand and manage their patient panel, focus on clinical care and patient relationships, and adopt attributed of the patient medical home model in British Columbia.

Self-Management BC/University of Victoria

Everyday, individuals with Chronic Health Conditions are undertaking activities to manage their own health.  These activities include implementing actions recommended by their physician and other healthcare professionals they see on a regular basis.  Some activities could include taking their medication as prescribed, eating balanced meals, exercising, and for their personal goals, they might include, getting quality sleep, dealing with difficult emotions and doing what they can to manage other issues with their chronic health condition(s).

Self-Management programs assist individuals in achieving those goals through the development of skills, such as Goal Setting (talking action), Problem-Solving, Decision Making and effective Communication Skills to name a few, to support the individual in becoming an effective Self-Manager enabling them to reach their goals.  Like any skill, active Self-Management must be learned and practiced.  

Self-Management BC programs focus on various Chronic Health Conditions, such as Chronic Pain (from whatever source), Diabetes, mental health challenges, heart disease, respiratory issues, Cancer,  MS, Parkinson’s and others.  To meet the challenges of COVID-19 Self-Management Programs now have various formats, from online computer based programs, the Zoom platform, phone-based and self-directed learning. 

ISSofBC

ISSofBC is one of the largest immigrant-serving agencies in Canada, with a national and international reputation in the field of migrant issues and services for immigrants and refugees. Every year, ISSofBC supports more than 20,000 people with a broad range of services through 14 offices located in Metro Vancouver and across BC.

Streamlined access to care is a major need among newcomers, especially those with significant language, cultural and financial barriers. ISSofBC has had a long-held vision to address this need.

With the support of selected community organizations with shared values, as well as funding support from government and the private sector, ISSofBC opened the ISSofBC Welcome Centre in Vancouver in 2016. The Centre houses 58,000 sq ft of residential units and support services delivered by ISSofBC and co-located community organizations. Building on the success of this model, ISSofBC opened the ISSofBC Welcome Centre in Surrey in 2019.

ARETE safety and protection

 

Founded in 1992, ARETE safety and protection Inc. is a Vancouver-based firm that specializes in the management of workplace conflict and the prevention of violence. Through nearly 30 years of experience, we have developed a unique training curriculum that plays a significant part in creating healthy and respectful workplace cultures for organizations across Canada. Our expert training specialists also provide coaching and direct feedback to participants as they learn to safely respond to both internal and external conflict. Threatening customer behaviours, verbal abuse and hostility are an unfortunate reality for many workplaces. Our approach recognizes that the level of risk is often determined by the choice workers make in response to the behaviours of customers and the public.

We train health care, federal, provincial and municipal governments, crown corporations, as well as not for profit and private sectors. Clients and their employees have expressed that ARETE training shows benefits not only in creating and maintaining a respectful workplace culture, reducing employee stress and absenteeism, but also in enhancing morale, productivity, and team atmosphere.

Workshops include: COVID-19 Customer Conflict Management Training, Violence Prevention and De-escalation Training as well as Respectful Workplace Bullying and Harassment Prevention Training. All workshops are customized to meet the job-specific needs of the participants by incorporating spontaneous and realistic scenarios.

Older Adult Community Mental Health Services (OACMHS)

Age - 65 years of age or older

Diagnosis – have a late onset major mental illness, or behavioral and psychological symptoms of dementia (BPSD).

Risk: present risk(s) in their current environment.

In addition, OACMHS may also serve individuals under 65 years of age who have behavioral and psychological symptoms of dementia.

Please note since we are a community service, our office hours are Monday – Friday 0830-1630.

Our service model is short-term about 3-6 months and may change case by case. 

We require GP to be on board as our services provided by our Geriatric Psychiatrist is a specialty service and we do require a referral from GP.

Process:

- GP makes a referral to our team seeking support, such as; assessment from our Geriatric Psychiatrist for medication management, and short term case management may also be provided by our clinician if deemed necessary.

- Once we have completed our assessments and treatment for the client, we close the referral accordingly and discharge client with recommendations back to care of their GP. 

We are not an urgent service, we have a process and it may take 2-3 weeks for follow up. 

The BC Settlement and Integration Services

The BC Settlement and Integration Services is a program of Options Community Services that provides support to refugee claimants, temporary foreign workers and international students in Surrey, Delta & Langley. 

South Asian Healing Network

SAH Network provides vital connections for South Asian youth to a community of peer learning and support. We seek to normalize healing by engaging in discussion and learning around topics of family dysfunction, problematic alcoholism or other substance use within our community, and mental health.

Mediworks Homecare is a private Home Support Company serving clients/patients in the Lower Mainland area.  We offer a variety of services including mental health which makes us very unique from other home support companies.
