MISSION   We engage and support primary care providers as collaborative and trusted partners to improve comprehensive primary patient care by strengthening the member voice and influence on innovative local healthcare delivery.

AIM   We foster a strong and healthy community of providers, improve the health of our communities, and enhance the quality of patient care and the primary care relationship.

 

 

OVERALL STRATEGIC GOALS FOR 2024/25

 

1. Strengthen member engagement and support provider wellness

   Enhance active participation and collaboration among members to develop and implement local healthcare solutions and support individual wellness.



2. Increase, retain and support patient attachment

   Enhance the capacity of primary care providers to attach new patients and increase access to timely appointments.



3. Support coordination of primary care services for unattached patients

   Strive, within the community’s available capacity, to provide all residents of the SOS, including those without a primary care provider, access to comprehensive primary care services.



4. Measure and communicate results

   Develop a robust system to measure the impact of initiatives and communicate results to stakeholders.



5. Prioritize organizational wellness

   Stabilize and strengthen the organizational wellness of SOS Division staff, Board, and key committees, ensuring a resilient and adaptive structure that supports our mission and vision.
