Home Health

To improve efficiencies and optimize patient access to home health support, each Langley family doctor now works with a single home health representative for all patients in their practice. Our initial home health focus was through BreatheWELL at Home, which helps patients with COPD, a chronic lung disease, manage their condition at home. Providing in-home care can help reduce both in-office doctor visits and hospitalizations.
 

Rapid Access to Psychiatry

The Division worked with Langley Mental Health and Substance Use to help provide patients with rapid access to psychiatric support.  Two clinics have opened since we began our collaboration with psychiatrists.
