As part of a Shared Care funded Chronic Pain project several resources have been developed to support physicians and their patients who suffer from Chronic Pain.
- Local Pain Team Program - A 12 week program that aims to improve patient functionality by improving their self-management capacity.

- Local Pain Team Intake Form - Intake form used by Local Pain Team nurse.
- Local Pain Team Role of Pain Specialist - Graphical representation of the role the Local Pain Team hopes to link with a Tertiary Centre.

- Chronic Pain Management Roadmap - A tool that contains many resources to support patients in improving their self-management capacity.
- Chronic Pain Functionality Scale - A patient friendly scale that focuses on progress in function using cartoons. This tool includes a journal and provides an overview of other activities patients might want to consider based on their functionality.

- Docs4You{th} - Youth Attachment Initiative Project Resources

The Docs4You{th} or Youth Attachment Initiative's aim was to connect local unattached youth with a primary care provider. The project was based on an example from Summerland, British Columbia. The process and tools were further developed with a team of local youth service providers, primary care providers and passionate local youth who wanted to make a difference in their community.
