Chronic Pain Project Resources

As part of a Shared Care funded Chronic Pain project several resources have been developed to support physicians and their patients who suffer from Chronic Pain.

Docs4You{th} - Youth Attachment Initiative Project Resources

The Docs4You{th} or Youth Attachment Initiative's aim was to connect local unattached youth with a primary care provider. The project was based on an example from Summerland, British Columbia. The process and tools were further developed with a team of local youth service providers, primary care providers and passionate local youth who wanted to make a difference in their community.

On November 7, 2016 Dr. Bernacki provided online video education about the serious illness conversation guide, a simple tool developed by Harvard Medical School to perform a goals of care discussion.
