Shared Care is a provincial partnership of Doctors and BC and the Ministry of Health that supports family physicians, specialists, and health care partners to work together to improve the coordination of care for local patients. By strengthening communication and collaboration across care settings, East Kootenay Shared Care initiatives help identify and reduce healthcare gaps, address inequities, and develop practical locally-developed solutions that enhance both the patient and provider experience. Since 2013, the East Kootenay Shared Care Committee has brought forward regional and community projects that foster stronger partnerships, expand shared expertise, and move the region toward a more connected, more efficient, patient-centred health system.
