PCN Support Staff 

The CR & District PCN is supported by a PCN Manager and a PCN Project Lead. They work with and support clinician team members in their patient care activities and in meeting the goals of the PCN. 

PCN Manager Stacy Folk | sfolk@crddivision.ca 

As PCN manager, Stacy Folk oversees day-to-day operations of the PCN and PCN team. She is the initial contact for PCN feedback from practices and PCN staff. 

PCN Project Lead Kirstie Stewart | kstewart@crddivision.ca

As PCN Project Lead, Kirstie Stewart provides support for the process, structure, documentation, and evaluation tools for the implementation of the PCN. She collaborates with the larger Campbell River & District PCN team and the PCN Operations Committee to support the facilitation and roll out of the PCN Service Plan.

As PCN Team Lead, Emily supports the Allied Health Professional team and works with key stakeholders to pursue goals of the service model.
