The Family Practice Services Committee (FPSC) introduced the Long-term Care Initiative (LTCI) in response to the growing senior population in British Columbia and a downward trend in the number of physicians providing services to seniors in long-term care.
Aim: To improve medical care for long-term care patients by enabling physicians to develop local, scalable, and sustainable solutions that meet the following five best practices expectations and three systems level outcomes.
Best Practice Expectations
- 24/7 availability and on-site attendance when required.
- Proactive visits to residents.
- Meaningful medication reviews.
- Completed documentation.
- Attendance at case conferences.
System Level Outcomes
- Reduced unnecessary or inappropriate hospital transfers.
- Improved patient/provider experience.
- Reduced cost/patient as a result of a higher quality of care.
