The provincial Long-term Care Initiative (LTCI) is a physician initiative designed to improve care for older adults in long-term care (LTC) homes.
The LTCI was launched in June 2016 and 12 LTC homes on the North Shore are now involved. Twenty seven family physicians and 2 nurse practitioners are participating in the program. The Division works collaboratively with Vancouver Coastal Health, physicians, nurse practitioners, and LTC homes on quality improvement initiatives that support patient care.
