Our GP/Specialist Relations Committee identifies collaborative opportunities between family physicians and specialists to improve referral processes and access to specialist care. We provide guidance and recommendations to current and future Shared Care projects. We also welcome new specialists in the community by sharing local community resources and tools, and informing family physicians on how to access their services.
GP/Hospitalist
Aim: Improve communication process between community and acute care family physicians at transition points in care

Our activities include identifying and implementing effective communication tools between community and acute care physicians at transition points in care in order to provide coordinated patient care. The GP/Hospitalist working group is currently in the process of developing e-notifications of admission and death from acute care to community physicians.
GP/Orthopedics
Aim: Improve access to and communication process between North Shore family physicians and orthopedic surgeons

The North Shore GP/Orthopedics Initiative started in 2013 in collaboration with North Shore orthopedic surgeons and Shared Care to improve access to orthopedic consultations and to improve communications between orthopedic surgeons and family physicians. This resulted in the development and implementation of the Rapid Orthopedic Consultation Clinic (ROCC) and Ortho Advice Line. Since the development of ROCC, we have seen a reduction in wait times from 18 months to four months. We continue to support our orthopedic colleagues in improving access, and to support our members in optimizing this invaluable service.
GP/Psychiatry
Aim: Increase client access to psychiatric services, support GPs in treatment of psychiatric conditions and to reduce the waitlists by providing community-based psychiatric assessment and consultation services

The North Shore Division of Family Practice and Vancouver Coastal Health (VCH) collaborated to establish the Psychiatric Consultation Clinic, which has been operating since 2012. In 2017, the GP/Psychiatry working group began a new project to improve communication and coordination between GPs and VCH's mental health and substance use services.
