Shared Care

Since 2006, the Shared Care Committee has helped more than 2,500 family physicians and 240 specialists to work together on over 240 projects across BC. Shared Care's goal is to support family physicians and specialists in working together to improve the flow of patients from primary to specialist care. Shared Care works towards understanding the working relationship between family physicians and specialists, develops collaborative approaches to address barriers to care, embraces new technologies, foster patient self-management and reduces unnecessary burdens on patients and their families. 

The Shared Care Committee is a joint collaborative committee between Doctors of BC (DoBC) and BC Ministry of Health working to improve patients' outcomes. The purpose of the initiative is to support family physicians and specialists in working together to improve the patient's journey through the health care system. The goal Shared Care is to:

  • Understand the working relationship between family physicians and specialists, while implementing pragmatic solutions to improve and support it;

  • Develop collaborative approaches that address barriers to care;

  • Embrace new technologies, service models, and payment methods;

  • Foster patient self-management; and

  • Reduce unnecessary burdens on patients and their families

Thompson Shared Care Steering Committee

The Thompson Shared Care Steering Committee is a Committee of the Thompson Region Division of Family Practice whose primary responsibility is to facilitate and oversee the development and progression of the Thompson Region Shared Care projects. The Committee works to address the strategic issues involved in establishing, implementing, and monitoring the local projects. The Committee strives to maximize the potential for success of the Shared Care program through coordinated and strategic implementation of the Shared Care initiatives, by maintaining an active Steering Committee that provides oversight, promotion, and supports projects. 

 

Membership of the Thompson Steering Committee consists of a mix of health care professionals and representatives including: 

  1. Physician Representation
    • Co-Chairs (one specialist and one family physician)
    • Three specialists
    • Three family physicians
  2. Thompson Region Division of Family Practice
    • Chair of Thompson Region Division of Family Practice
    • Executive Director
  3. Interior Health
    • Medical Administration from Acute and Community
  4. General Practice Service Committee/Specialists Service Committee
    • Shared Care Committee Sponsor
