events and initiatives. The Division appreciates the ongoing level of commitment to work
together in health for Langley.

PATIENT MEDICAL HOME

LDFP is moving forward with the Patient Medical Home model in Langley.
 

Watch for LDFP member event coming soon.

The Division will be bringing members together to strategize around the PMH model and goals for Langley for the next 24 months. 
PHYSICIAN WELLNESS

In January, over 100 GPs/NPs, Specialists and new Langley Hospitalists joined together for an amazing Wellness Evening! 
 


With improv entertainment provided by the Rock Paper Scissors group there was much laughter around the challenges of daily life as a health care provider.
Later, stories were shared and groups talked about how to support each other and maintain physician wellness.
 
We uncovered the need to find access to primary care for our Langley Members.  Those without their own physician are encouraged to call:

 Youth Hub   

Door prizes from the January Wellness Event included 4 baskets made up of youth essentials, such as personal hygiene items, socks and underwear, healthy snacks and transit passes.

The baskets valued around $150 each were donated to the Youth Hub in name of the door prize winners. These items go a long way in supporting youth and the Hub is always appreciative of any donations.

         
    

We are excited to announce the opening of the new Langley Youth Resource Centre in July 2017.

The Youth Hub is looking for additional primary care providers interested in working in the Hub a few hours a month, providing care to the youth.

RESIDENT NIGHT - Pearls of Wisdom


In February, over 50 members and  guests came together to mingle for a night of craft beer tasting, delicious food, and story sharing. GPs passed on many Pearls of Wisdom for the Residents to consider as they move forward in their career.

Some valuable "pearls":
  • Take care of yourself, so you can take care of others
  • Remember the person is infinitely more important than the illness, look after the person
  • Common things are still common, trust instinct, rule out the worst first
  • Don't take your work or complex patients home
LDFP commits to keep updating and sharing this collection for years to come.
MedWatch - Health Care Emergency Program

LDFP continues to spread the MedWatch Health Care Emergency program community wide. With our Firefighter partners we are educating local seniors at community events that will continue in the coming weeks. To date close to 1500 packages have been distributed.

We encourage seniors to first complete the package with family and then make an Advance Care Plan appointment with their GP/NP to complete the MOST/DNR form.


The MOA network will be kept up to date on upcoming community dates.
 
MedWatch package: patient information form, medication list, MOST formDNR form and Advance Care Plan documents, kept in the Fraser Health Greensleeve. 2 Green dot stickers included for location markers.

Seniors LINC 

The LINC IP Team has been accepting patient  referrals from the prototype practices.

LDFP  is working with LINC to gather stats and information to monitor the referral process and integration of the IP Team with GP practices.

Residential Care Initiative

There were 2 Polypharmacy Risk Reduction Education sessions in the month of February, with presentations from Shared Care, Dr Peter Methven and a facility pharmacist.

The next meeting is scheduled for:

 Immigrant access to Primary Care

Thank you to Drs. Beth Watt, Leo Wong and Yassir Alaaraji  who recently presented to a group of newcomers to Langley on how to access primary care.

Further sessions are planned in partnership with LCSS
