Our Primary Care Network (PCN) is focussed on expanding team-based care with the addition of Neighborhood Nurses, Neighbourhood Social Workers, Patient Educators, Pharmacist, and counselling to form a team around Family Physicians in White Rock-South Surrey. Some Clinicians work directly with Family Physicians and their patients and some develop program resources.
