Victoria Divisions of Family Practice

Victoria Primary Care Network

Across BC, Divisions of Family Practice, health authorities, First Nations and community partners are working to establish Primary Care Networks.

WHAT IS A PRIMARY CARE NETWORK?

A Primary Care Network (PCN) is a geographic area where family physicians and nurse practitioners, nursing and allied health providers, and health and community organizations work together to plan and deliver primary care services.

WHAT AREA DOES THE VICTORIA PRIMARY CARE NETWORK SERVE?

PCNs operate in defined geographical boundaries. The Victoria PCN serves patients and populations across Urban Victoria including James Bay, Fairfield, Oaklands, Fernwood, downtown Victoria, Vic West, Oak Bay, Gordon Head, Shelbourne, Interurban, Tillicum, Quadra and Swan Lake.

WHO IS A PART OF THE VICTORIA PRIMARY CARE NETWORK?

There are 50 primary care clinics (also known as Patient Medical Homes) with over 140 family physician and nurse practitioner members participating in the Victoria PCN.

Victoria PCN healthcare providers collaborate with patients, families, caregivers and community services to design and provide comprehensive, patient-centered, culturally informed healthcare.

PCN healthcare professionals enhance patient care using a team-based approach.

Together, they:

  • Support each other and work to their strengths.
  • Further link patients to other parts of the system, including health authority specialized community programs for vulnerable patient groups (e.g., frail elderly)
  • Collectively increase a community’s capacity to provide greater access to primary care.

WHAT SERVICES DOES THE NETWORK PROVIDE?

The Victoria network is being designed to adapt and respond to the specific needs in the community and to strengthen services identified as high priority.

The Victoria PCN was announced in July 2021, and is in year two of a four-year implementation plan to recruit 96.5 more full-time health-care providers.

This will include:

  • Family Doctors
  • Nurse Practitioners
  • Registered Nurses
  • Mental Health Substance Use Clinicians
  • Physiotherapists
  • Clinical Pharmacists
  • Indigenous Wellness Advocates
  • Social Workers
  • Registered Dieticians
