The Victoria Primary Care Network is seeking an applicant(s) to join the region in providing primary care services to underserved populations. We seek New to Practice Family Practitioners and/or several experienced Family Practitioners to help stabilize primary care supports for Newcomers to Canada. The position is 1.0 FTE which can be either one individual or more than one, adding up to 1.0 FTE, who would share the practice. 

The Family Practitioner(s) will provide full service primary care for patients coming from the federal newcomers to Canada program who need stabilization prior to transfer to a regular primary care provider, including federally-mandated Health Assessments. The practice will be made up of children, adolescents and adults. The population served includes immigrants and refuges from Syria, Africa, Greece, Lebanon and Egypt. Translation services are available. There is a focus on mental health. Knowledge of trauma informed practice is essential. The Family Practitioner will have support from a Primary Care Network team, including Mental Health Workers and Pharmacists.

  • 1.0 FTE coverage (either one individual or more than one, adding up to 1.0 FTE, who would share the practice)

  • Open to new to practice physicians and/or physicians currently practicing as FPs, ERPs or Hospitalists.
  • Shared philosophy of trauma-informed, culturally-safe care essential.
  • Experience in hospitalist, rural or ER would also be considered assets but not required.
  • Interest in innovation in primary care and working part-time/shared position.

Cardiometabolic Collaborative Clinic

The general practitioner would be trained on how to manage specific areas of chronic disease management and be responsible for helping to develop and run additional group medical visits facilitated by nurses and other educators as appropriate. Geriatric Specialty Services Senior's Outreach Team (Perm/PT)

Great group of 3 Geriatric specialty service GP's looking for a fourth GP to join the Senior's outreach team. We are looking for a GP to support the Esquimalt Westshore Health Unit working with a multidisciplinary team including nursing, social work, occupational therapist and Geriatric psychiatry support.  This is a flexible part time position of approximately 3-5 sessions per week.  You are expected to attend weekly rounds ( one hour on Thursdays) and the other sessions are spent doing home visits  (booked on your schedule/availability) and conferencing with your care team.  Geriatric psychiatry support is available weekly at rounds where we review cases and new referrals as well as by phone/email as needed.   Your role working with support from the interdisciplinary team is providing assessment, diagnosis and management plans/care planning for older adults with cognitive impairment, neurocognitive disorders and mental health conditions.  A family physician with experience working with seniors or certificate of added competence in Care of the Elderly would be ideal.  Training will be provided by current GP group as well as EWHU clinical team, and monthly education rounds are available for GP group with geriatric psychiatry for additional support.

Feltham Medical Clinic (PT)

After building a practice in Victoria over 25 rewarding years, Dr. Janicki in relocating to the mainland. His 1850 patient panel is available immediately with the current plan to stay until a replacement is found. Dr. Janicki works with three collaborative colleagues who share the staff, the office space and overhead.  Dr. Janicki currently works five days a week; one and a half days in the office where he has access to two exam rooms, and three half-days remotely, seeing 20-25 patients per half day. Partners in Canadian Veteran's Rehabilitation Services (PCVRS) provides comprehensive rehabilitation focused assessment and intervention for Canadian Veterans who are experiencing barriers transitioning to life after service due to service related injuries or illness. Using a biopsychosocialspiritual approach, the assessment and intervention services are performed by an interdisciplinary team of clinicians with expertise in the management of complex conditions such as chronic pain, persistent symptoms following head injury, MSK injuries, often with comorbid mental illness with presenting psychosocial barriers to recovery.

As part of the interdisciplinary team the Physician shall use evidence and measurement-based care to perform the following:

  • Complete a thorough assessment of presenting eligible health problems;
  • Perform medication review and identify drug therapy problems; 
  • Clarify relevant medical issues including comorbid conditions;
  • Clarify diagnosis and prognosis related to eligible health problems; 
  • Identify relevant accommodations or medical restrictions;
  • Identify and provide rationale when additional specialized assessments are required to enable achievement of rehabilitation goals; 
  • Collaborate on interdisciplinary recommendations for treatment with an emphasis on education, coping and self-management to improve function and recovery;
  • Collaborate with the Veteran to promote understanding of relevant medical conditions and recommendations
  • Communicate directly, through targeted outreach, with the Veteran’s  primary health care provider to collaborate on rehabilitation care;
  • Care may be provided in-person or via virtual care
  • Engage in continuous quality improvement, learning and skills development

Pulse Complete Cardiac Care (a division of RebalanceMD) opened in 2022 with a vision to change the experience for cardiac patients on Vancouver Island by creating an outpatient centre of excellence for integrated care.

Dr. Neil Crofton: Panel available after retirement

I am retiring as of April 30, 2024. My practice of 35 years comprises approximately 1000 patients and includes all age groups, from babies to nonagenarians. I see 25-35 patients per day (currently about 60% in-person and 40% phone visits). My practice includes about 25 OAT patients, although they should be able to access OAT services from one of the OAT clinics if necessary. Under the new longitudinal family physician payment model, expected annual income is estimated to be around $500,000. 

I'm currently sharing an office with another FP who plans to retire as of April 30, 2024. The office is set up for 2 MOA stations, 2 FP offices, 2 exam rooms, a kitchen/break area, a utility room which can function as an extra exam room which has an autoclave, and a storage room for charts and supplies, etc.

High Complexity Care Team Physician Lead - EOI

The Victoria Primary Care Network (PCN) is seeking applicants for a Physician Lead to explore and develop the implementation of a High Complexity Care Team (HCCT) in Victoria. The HCCT is a physician-lead initiative in collaboration with the Victoria PCN, Ministry of Health, Island Health, Divisions of Family Practice, and Indigenous Health Collaborative. The Victoria HCCT will be modelled after the Eagle Creek HCCT which has been operating since January 2019, serving the medically and socially most complex patients in the Westshore, Sooke and Esquimalt Regions.

The Victoria HCCT will be a multidisciplinary team that will include Family Physicians, Nurse Practitioners, Social Workers, and a Registered Nurse. The Physician Lead will be compensated hourly at the current physician sessional rate to a maximum of 20 hours per month. We are looking for a physician lead to develop the Victoria HCCT and envision the team to be fully functional in the second quarter of 2022. If you are interested in innovative primary care delivery to our most complex patients, we want to hear from you.

Key Points:

  • The HCCT will follow the model piloted in Westshore, attaching highly complex, resource intensive patients not otherwise served by the local system. Appointments are available within one business day, virtually and within one week in person when clinically indicated. The services of the clinic will reduce acute care utilization and provide a stable provider/team relationship with these patients.
  • The successful candidate will need to review the Westshore HCCT model and adapt the model to work within the existing health care structures in Victoria.
  • Will require strong stakeholder engagement activities, including liaising with Victoria/Saanich Health Unit, Privacy Officers, and Inpatient Leadership at the Royal Jubilee Hospital.
  • Responsible for creating a base referral process using clear patient profiles.
  • Responsible for creating a sustainability plan for the Victoria HCCT.
  • Determines the linkages between Westshore HCCT where appropriate.

What we are looking for:

  • Dedicated, hard-working and collaborative physicians.
  • Ability to work as a team with Nursing, Nurse Practitioners, and Allied Health Professionals in a non-hierarchical manner essential.

BC Service Contract | Island Sexual Health Community Health Center

Island Sexual Health Community Health Centre (ISH) is seeking Family Practitioners to work collectively with an interdisciplinary health care team at Island Sexual Health to provide a blend of longitudinal primary care services and focused sexual health services, along with services that address the social determinants of health to populations residing in Greater Victoria, Saanich, and the Western Communities.

ISH is a Community Health Centre (CHC) that provides wrap-around sexual health care, longitudinal care for priority populations, and allied health care.

The Family Practitioner is a contracted provider (rate based on current service contract) with ISH and will fulfill the obligations as outlined in the service contract including supporting patient attachment for primary care in accordance with Ministry of Health requirements for panel size and composition. The Family Practitioner will also provide episodic care for sexual health for both attached and unattached patients.

  • Shifts rage from Monday to Saturday and two evenings (variations of shifts)
  • After hours care for patients attached to the practitioner is inherent in the services
  • Member of on-call group; expected call rotation
