Our journey to building networks of primary care in Surrey-North Delta has reached a new milestone: the launch of our Primary Care Network Governance Structure.
This includes equal representation from Fraser Health and Physicians, with an overall steering committee to guide the implementation of primary care networks generally in Surrey-North Delta, and local steering committees to represent and respond to the needs of individual neighbourhoods.
We also have a Patient Medical Home Working Group, whose purpose is to gather and champion the voice of Surrey-North Delta family physicians.
A visual representation of this structure is below. Orange bubbles represent Fraser Health groups, red represents Primary Care Network groups with equal representation from Fraser Health and Family Physicians, and Blue represents Family Physician groups.
Physicians from each neighbourhood have stepped forward to represent the local physician perspective. We encourage you to reach out to your neighbourhood leads as we continue our journey towards Primary Care Networks.
