What Is a Primary Care Network?
A Primary Care Network (PCN) is a network of healthcare clinicians working with patients and families as a team to support their health care needs and to deliver medical care beyond what the traditional Canadian family practice model has offered. In addition to patients, the Shuswap North Okanagan PCN team is made up of family physicians, nurse practitioners, registered nurses, occupational therapists, social workers, registered dietitians, physiotherapists, pharmacists, and others.
The goal of the Shuswap North Okanagan PCN is to strengthen and support primary healthcare so that everyone in our region can access the care they need. By working closely as a team, PCN patients and families will receive clinic, community, and Interior Health-based services in supportive, inclusive and safe environments.
The Shuswap North Okanagan currently has two separate PCNs based on geographical areas: Salmon Arm and Vernon.
