Long-Term Care Initiative

In 2015, long-term care facilities, across the province, were having difficulty finding family physicians to take on the care of newly admitted residents. In response, the BC Ministry of Health launched the Long-Term Care Initiative (LTCI) to standardize medical care in BC’s long term care facilities and attract more physicians to the work. Any physician who participates in the LTCI (a.k.a. an LTCI Physician) must agree to provide the five best practices of care and promote three system level outcomes:

Best practice expectations: System level outcomes:
  • 24/7 availability and on-site attendance, when required
  • Proactive visits to residents
  • Meaningful medication reviews
  • Completed documentation
  • Attendance at case conferences
  • Reduced unnecessary or inappropriate hospital transfers
  • Improved patient-provider experience
  • Reduced cost/patient as a result of a higher quality of care

Richmond’s LTCI program launched on May 1, 2017 and is led by the Richmond Division of Family Practice, in close collaboration with VCH.
