Long-Term Care Initiative
The FPSC Long-Term Care Initiative was created to improve care for the ever growing senior population, and to support divisions of family practice in finding local solutions to improve care for these vulnerable patients.
The initiative was prototyped by five divisions, beginning in 2011: Abbotsford, Chilliwack, Prince George, South Okanagan Similkameen, and White Rock-South Surrey. Based on the successes achieved in the prototype communities (whose stories are below), the FPSC has earmarked up to $12 million annually to support divisions in designing local, scalable, and sustainable solutions that cover all long term care facilities in about 104 BC communities.
For more information, view the Long-Term Care Initiative resources on the JCC Resource Catalogue. A series of infographics displaying early results from the prototype communities is available on the FPSC website.
