Gaps in communication or referral processes between family physicians and specialists can impede the flow of care for patients and families. The Division strives to work collaboratively with North Shore specialists to identify gaps and solutions to improve patient care on the North Shore.

Family physicians and specialists also have an opportunity to work together on Shared Care Projects to improve the coordination and delivery of healthcare.
