Long-term Care Initiative

The Family Practice Services Committee (FPSC) introduced the Long-term Care Initiative (LTCI) in response to the growing senior population in British Columbia and a downward trend in the number of physicians providing services to seniors in long-term care.

Aim: To improve medical care for long-term care patients by enabling physicians to develop local, scalable, and sustainable solutions that meet the following five best practices expectations and three systems level outcomes.

Best Practice Expectations

  • 24/7 availability and on-site attendance when required.
  • Proactive visits to residents.
  • Meaningful medication reviews.
  • Completed documentation.
  • Attendance at case conferences.

System Level Outcomes

  • Reduced unnecessary or inappropriate hospital transfers.
  • Improved patient/provider experience.
  • Reduced cost/patient as a result of a higher quality of care.
