Jordana Moxin
Attachment Coordinator
Jordana is the Attachment Coordinator for the division. She was brought on to our team to help patients that do not have a family doctor get connected to doctors accepting patients. She regularly updates and maintains our internal registry of unattached patients within the areas that our division oversees with the goal of attaching patients to Community Family Physicians and Nurse Practitioners whenever possible. Her job also requires developing and maintaining relationships with healthcare professionals within the community and assisting with projects that are related to the attachment of patients and Family Doctors.
Jordana graduated with a diploma in Community Support and Social Services, which quickly opened doors to the world of healthcare. Outside of work, she enjoys spending time with her daughter, being out in nature, and expressing her artistic side through writing and sketching.
