Collective Impact and Collaboration
The Cowichan Division of Family Practice made a presentation to the General Practice Services Committee (GPSC) on December 13, 2016, highlighting the ways in which relationship building has led to our division’s success in engaging the community in two large scale transformation change initiatives:
- Patient medical home/primary care home (Phase 1: Frail Seniors, Phase 2: Social Determinants of Health).
- Housing First initiative to support shared MHSU strategic priorities.
Through use of a collective impact framework and partnership alignment principles, the division has been able to evolve our CSC, identify shared priorities, and ensure that these projects took off quickly and successfully.
