CardioMetabolic Collaborative Clinic Launch

We would like to invite referrals to the Cardiometabolic Collaborative Clinic (CMCC).  This is a primary and secondary vascular prevention clinic.  Our clinic systematically manages key cardiometabolic abnormalities (diabetes, fatty liver, dyslipidemia, hypertension) to optimize cardiovascular risk and to get patients to target while optimizing use of pharmacotherapy to reduce overall risk as indicated while supporting lifestyle change.

Currently our clinic is comprised of two endocrinologists, a cardiologist, hepatologists, internists, and an electro-diagnostics unit.  We also work collaboratively with general practitioners and allied health care professionals (dietitians and diabetes educators) in achieving our mission of comprehensive risk factor management for at risk patients.

Movement Disorders Clinic Announcement

Dr. Keiran Tuck, Neurologist and Movement Disorder Specialist began seeing patients at the new Movements Disorder Clinic at RJH in Victoria in May 2019. This service is expanding to include a satellite Telehealth clinic out of Nanaimo- NRGH in October. Patients who choose to be seen in Nanaimo will continue to be under the care of Dr. Tuck but will meet with him over Telehealth.

OSCAR Patient Summary Template Now Available

The seasonal influenza vaccine will be delivered to BC in early October.

Transitions in Care has partnered with PSP to build an EMR-specific template for OSCAR users, making it easier for family physicians to quickly gather important information to include in patient summaries, which can be sent to Island Health in response to a patient admission, or proactively for patients that physicians deem to be at high risk of hospitalization in the future. This means more time can be spent reviewing and providing information to best support acute care stays, instead of copying and pasting manually to populate the basics. Our website has a link to the downloadable template, installation instructions and helpful hints for filling in the templates. Check it out here.

For more information about sending in patient summaries or about the work Transitions in Care is doing to support coordination across patient transitions, please contact Kristin at katwood@divisionsbc.ca.

Vancouver Island IBD Clinic - Crohn's Disease Referrals

Symptom-based management of inflammatory bowel disease patients is no longer standard of care. There is a substantial disconnect between symptoms and inflammation in Crohn's disease. This differs from ulcerative colitis. In ulcerative colitis, rectal bleeding is predictive of colonic ulcers.

All Vancouver Island patients with Crohn's disease should be referred to the South Island Gastroenterology Central Access and Triage for assessment. (fax: 1-888-398-7091)

Notice from the Regional Pain Program - Referral Form Change

Scents Sign_1.jpg

The Regional Pain Program at Royal Jubilee Hospital has a provider with extreme sensitivity to all types of smoke (tobacco, cannabis, vaping) and scents.

Please inform the clinic if your patient smokes and advise them of the info attached prior to their visit to the clinic.

We have changed our regional referral form and added smoking for this reason.

Through a consultation with community GP’s we have also made a number of improvements to our referral form and it aligns with new community GP form standards.

Please indicate patient’s smoking status on referrals to the pain clinic until we can arrange a new pdf and EMR version of the referral form.

Thank-you for assisting us to provide safe care for your patients and our providers.
